These terms and conditions (terms) set out the basis on which you will be provided with treatment at any AJ Primus Healthcare Ltd facility.

AJ Primus Healthcare Ltd is not for use in medical emergencies or in situations where it is believed there is an urgent need for care/investigation. We do not provide services Out of Hours.
Charges
The charges for our appointments are available by contacting us by email or phone. Prescriptions and telephone consultations/discussions with the doctor incur charges; these and their charges are available by contacting us by email or phone. For treatment/services that you are paying for yourself, AJ Primus Healthcare Ltd will always provide you with the cost of the appointment prior to seeing the clinician.
Please be aware that there are some costs that can only be applied after seeing the clinician. These include but are not limited to:
- the cost of blood, urine or stool tests
- the cost of any imaging investigation or other further investigation.
There will need to be a follow-up appointment to discuss the results, the cost of which will be provided to you.
Before any test is carried out, the clinician will explain and describe the costs (where known) of any investigation carried out by AJ Primus Healthcare Ltd. If the test is to be carried out by an external organisation, for example an x-ray carried out by a hospital or diagnostic centre, the AJ Primus Healthcare Ltd clinician will try to offer the cost of this test but may not be able to do so. The costs of these tests will be available from the organisation who will be carrying out the test and we strongly advise you to contact then prior to the test being carried out to clarify the costs.
Cancellations:
We do wish to support all our clients healthcare needs.
Where appointments are cancelled at short notice or if patients do not attend their scheduled appointment other patients are unable to be assisted in their healthcare needs. Therefore, we request that if you need to rearrange the appointment you have booked, we would be grateful if you could give as much notice as possible.
We can reschedule your appointment at any time, without penalty or administration fees, provided a minimum of 24 hours notice is given. Should your appointment fall on a Monday we would expect contact by email to info@privategpextra.com or an answerphone message be left on the contact numbers 01614284464 or 0730617899 during the week-end (with a minimum of 24 hours notice).
In the event that you give less than 24 hours notice for rescheduling or cancelling an appointment, you will be liable for the full cost of the appointment, which is charged as a cancellation fee as our doctors will be unable to help other patients at such short notice.
Please note non-attendance of an appointment will be invoiced at 100% of the cost of the appointment and is to be paid within 15 days of invoice.

Complaints Policy and form
Insured Patients
This section will apply if you are covered by private medical insurance.
Please note that private medical health insurance rarely covers Private GP charges both for the appointments and investigations.
1. You agree to pay for your Care.
2. Whilst you will remain responsible for the payment of your Care, where you have private medical insurance:
- we will, where possible, process the insurance claim for your Care with your insurer, provided you have given us and your insurer all the information we and your insurer need to do so. If this information is incomplete or inaccurate, we may not be able to process your claim and you will need to pay for your Care, as set out in (c) below;
- where we process your insurance claim and your insurer pays us direct, the rate agreed between AJ Primus Healthcare Ltd and yourself will apply to your care. We do NOT charge a surcharge for care covered by an insurer or external organisation;
- if your insurer fails to settle our invoices (or any part of them) within 30 days of the date of issue we will investigate and if the outstanding amount will not be paid by your insurer we may invoice you directly or debit the relevant balance from your credit or debit cards in accordance with the process set out in paragraph 7 below (as relevant); and
- if we invoice you for your care or an element of it, you agree to pay us the amount invoiced within the time limits set out therein. If you do not think that we have invoiced you correctly, please let us know as soon as possible so we can deal with any mistake or misunderstanding. In any event, this must be done within 30 days of the invoice.
3. It is your personal responsibility to confirm with your insurer in advance that your care is covered by your insurance policy. AJ Primus Healthcare Ltd is not permitted to and will not obtain any such confirmation on your behalf. When you attend for treatment, if you want to check with your insurer whether any aspect of your care is covered by your insurance policy, we will give you access to a telephone so you can contact your insurer.
What happens if I suffer complications?
While the Clinicians will do their best to ensure a satisfactory outcome, no clinical procedure is entirely risk-free and the results of any particular treatment cannot be guaranteed with complete certainty.
Other Terms and Conditions: Private Patients
This section applies to all Patients
Clinicians (your attention is particularly drawn to this paragraph):
While at any AJ Primus Healthcare Ltd facilities you will be under the Care of the Clinician with whom you have been arranged an appointment.
Clinicians involved in your Care are independent practitioners and are not employees of AJ Primus Healthcare Ltd. Accordingly, AJ Primus Healthcare Ltd will not be liable for any act or omission of a clinician (or the company or partnership that employs or engages the Clinician). The Clinician will be responsible for the Care he/she gives you.
Debit/Credit Card: Debit card is requested as form of payment. It is requested that a credit card is not used. Company credit cards are not an accepted form of payment. Amex is not an accepted form of payment.
Overseas Patients
This section applies to all Patients who are not ordinarily resident in the UK.
If you are not ordinarily resident in the UK you will be liable to pay charges if you require NHS treatment whilst in the UK, whether related to your care from AJ Primus Healthcare Ltd (for example if you require treatment that AJ Primus Healthcare Ltd does not agree to provide) or not.
By agreeing to these Terms you confirm that you have to leave to enter the UK and that you meet all relevant immigration criteria. You also confirm that you have made adequate arrangements to pay for your care. AJ Primus Healthcare Ltd may contact the Home Office or UK Border Agency (as relevant) to the extent necessary to clarify any information regarding your leave to enter or remain in the UK in connection with your care.
Other Terms and Conditions: All Patients
Your Contract with AJ Primus Healthcare Ltd
By agreeing to the appointment you agree to be bound by these terms and conditions. If there is any conflict between these terms and conditions and any subsequent documentation that you sign on behalf of AJ Primus Healthcare Ltd these terms and conditions will take precedence.
AJ Primus Healthcare Ltd may update these Terms from time to time, however, any changes will only apply to any new episode of care that you receive. You agree to be bound by the terms and conditions in force at the time you attend the clinician. Note that any new terms and conditions will apply to any treatment Package which is part-way through when the terms and conditions are changed.
AJ Primus Healthcare Ltd rates:
Unless discussed prior to the consultation, the AJ Primus Healthcare Ltd rates will apply to your Care. If you want to check how much something costs, please ask. You can always check AJ Primus Healthcare Ltd rates by contacting us.
Cancellation of your treatment by AJ Primus Healthcare Ltd
AJ Primus Healthcare Ltd may need to cancel your appointment due to clinical or non-clinical reasons.
We will provide you with a reason why the cancellation was necessary. Where this is the case we will give you as much notice as possible. We will rearrange your appointment as soon as possible and agree on a date with you.
Notices and your contact details:
It is important that you keep us updated with any changes in your contact details.
Your property:
AJ Primus Healthcare Ltd facilities can be busy environments. While we will take all care to ensure the safety of your belongings, AJ Primus Healthcare Ltd does not accept any responsibility for the theft or loss of, or damage to, any of your or your visitors’ property.
Third Party Rights:
Except for you or AJ Primus Healthcare Ltd, no person, company or organisation will have any rights under or in connection with these terms and conditions. You agree to indemnify AJ Primus Healthcare Ltd against any claims brought by a third party in connection with your care.
Law and the Courts:
These Terms are governed by and shall be construed in accordance with English law and the courts of England and Wales shall have non-exclusive jurisdiction.

We reserve the right, at our sole discretion, to update, change or replace any part of these Terms and Conditions by posting updates and changes to our website. It is your responsibility to check our website periodically for changes. Your continued use of or access to our website or the Service following the posting of any changes to these Terms of Service constitutes acceptance of those changes.
Definitions:
“Applicable Law” means any and all laws, regulations, guidelines and professional obligations applicable to the provision of Care or the performance of services for you, including the requirements as regards treatment, procurement, research and storage of reproductive material;
“Care” means care, treatment, diagnosis, services (including Sundry Items) and goods provided by us;
“Clinicians” means all clinicians involved in your Care;
“Contract” means these Terms and conditions
“AJ Primus Healthcare Ltd”, “we” or “us” means (a) AJ Primus Healthcare Ltd
“Terms” means these terms and conditions;
